Please note that your application is not complete until the following documents are received:

For Vehicle Placement Requests:

• Proof of current driver’s license
• Proof of two months of income OR proof of school enrollment
• Your official Motor Vehicle Record (purchased for $9 through the Colorado DMV)

For Vehicle Repair Requests:

• Proof of current driver’s license
• Proof of current vehicle insurance.
• Proof of current vehicle registration with your name on it.
